At Northwest Hospital, providing excellent inpatient care is a core part of our mission. That’s why we’ve gathered an expert team of board-certified internists who are dedicated specifically to caring for patients in the hospital. Northwest Hospital Inpatient Team physicians share the care of inpatients with their primary care physicians, who may not be able to be at the hospital continually to direct their patients’ care.

The Northwest Hospital Inpatient Team provides 24-hour-a-day care, visiting patients daily, reviewing lab and test results and discussing diagnoses and long-term treatment plans with the patient, family and primary care physician. The Inpatient Team also works closely with nurses, physical therapists, social workers, dietitians and other staff to coordinate comprehensive care and treatment for each patient.

When patients are ready to leave the hospital, the Inpatient Team coordinates discharge planning, communicating and consulting with the patient’s primary care physician and other specialists to ensure appropriate follow-up care.
